For anyone who has had their jpouch removed, who did it and where? After 8 surgeries and many procedures we are likely looking at attempting to put an end to a miserable 3 1/2 years for our daughter. It's what she wants. She wants to inquire about the k pouch first though. Can you have the pouch removed, an end ileostomy done and then later try the k pouch I wonder?

How were your results? I know one person on here had such a rough time with healing.

Thank you!

I am sorry to hear about the problems your daughter has had with her jpouch. My situation was different (failed jpouch after 30 years), but my options were the same. I opted for a BCIR (modification of a k pouch) since I did not want an end ileostomy. The hardest part of the surgery, so I am told, is removal of the jpouch, but that plus creation of the BCIR (or k pouch) is done in one operation.

Everything went well with my surgery and I had a complete recovery three months after returning home. I did have pouchitis and some valve leakage for several months post-op, but that cleared up completely and has not returned. Although you can have the ileostomy first and convert to a k pouch or BCIR at a later time, that would involve two separate operations. I had my BCIR done by Dr. Ernest Rehnke in St. Petersburg, FL and received the best of care from him and the hospital staff. You can visit their website at www.bcir.com for more information. Please feel free to send me a PM if you would like more information, and best of luck with whatever you decide to do.

Hi! I'm so sorry that this may be the outcome for your daughter.

I've posted on some of your other questions, so maybe I'm repeating myself. I've had my pouch removed and now have a perm ileostomy and am doing so well. I healed really well; recovery for totally straight forward. God was merciful to me for sure! My surgery was done by Dr. Jonathan Efron at Johns Hopkins Hospital (Baltimore). Recovery was a solid 6-8 weeks, but now I have no symptoms or issues or any drugs to take. Fistulas are healed!

K pouch sounds like a good idea, but I wasn't a candidate for it because of my Crohn's diagnosis plus all the other issues I've had...GI just said I'd be dealing with more issues with a new pouch. I've had my share of abdominal surgeries and I think the idea was go in one last time and be done with it. My surgeon wasn't even willing to try a temp ileostomy on me since he knew the fistulas would heal best without the pouch (and that inflammatory mass). Such a hard decision! But I am happy with my decision as I tried everything I could before deciding on excision.

Best wishes as you explore all the options!

Thank you for the response. Yeah she has had 8 abdominal surgeries in 3 years with more to come. That doesnt count the minor 2 she had not to mention all the exams under anesthesia. And the chronic abdominal pain she has now. She does not form adhesions at all and I worry that will make her a poor candidate.

May I ask how the healing of your bottom went? She is hoping to get this done by the end of the year so she can start classes mid January. Two are online and one is at the local college 4 days a week but only 1 hour a day.
I was in the hospital for the customary 21 days for the j pouch removal and creation of the BCIR and my bottom was completely healed when I was discharged. I had no discomfort there during the 12 hours (sitting most of the time) trip home. In response to your other post, I have read posts by others who had their jpouches left in and a temporary or end ileostomy created with no necrosis. I would ask her surgeon about the advisability of attending classes in mid January. In my opinion, the online courses would be no problem, but I would not take the on-campus course so soon after surgery.

She has a surgery date of December 26th! She is relieved. She is not interested in a continent ileostomy after she found out more information. Personally I don't think she would be motivated enough to follow all the post op instructions. Change her loop to an end, take out that pouch and away we go! Oh and sew up the bottom. Smiler. They are saying a three day stay. That i will have to see to believe!! Haha. She is notorious for complications.

Interestingly our surgeon said to not do the BCIR because they remove a LOT of them. he said the kpouch or tpouch would be a better option, but they come with high complication risks. Not something we need.
