Hi all! I’m 10 months out from takedown. Everything is pretty great for me. No meds. No bleeding. No pain. I go about 4 times a day. I only have one issue....Straining!!! Drives me crazy. What I don’t understand is that it’s not every time though? A lot of times I sit and push a little and I go fully and all is well. But some random days I feel so uncomfortable. I feel like I need to go and it feels low but when I go I am straining so hard and not much is coming out. I try over and over again all day long and feel crumby. I usually have to take MOM or a magnesium pill. Sometimes it works and sometimes it doesn’t. Any ideas what this is? I do nothing different on those days food wise. Not sure why this happens. Straining does tend to be a problem for me a lot of times but some days are waaay worse.

Unless what’s coming out is hard then this isn’t conventional constipation, and laxatives probably won’t help very much. Are you drinking plenty of water? What is the consistency of the stool? You could have a prolapse, which is when part of the pouch moves to block the exit when you bear down, or it’s also possible that your sphincter is tightening when you mean to be relaxing it. Four times a day is relatively infrequent for a J-poucher, which may be fine for you, but you might want to experiment with a bit higher frequency, perhaps 6 times per day, and see if your pouch empties easier when it’s less full. Do you have a good gastroenterologist you can consult?
