does anyone have trouble emptying the pouch? It'll feel like you have to go and if you don't reach the toilet in that 15 second time frame then your stomach gurgles and all of the sudden you can't get anything out? My surgeon prescribed Nifedipine/lidocaine ointment (same idea as diltiazem) for use four times per day to help relax the sphincter muscle which may be tightening but I'm curious as to how people have handled it. 

Due to this over-tightening, I have also had trouble working out such as running, biking, and, and lifting weights. Every time I do those activities I feel like I have hemorrhoids for days but really it's the anal area tightening and subsequently ill get anal fissures from there which don't resolve for a week or two. Sometimes Dr. Herfarth, a clinician (non-surgeon) who solely works with J Pouchers at UNC will give me a short course of cipro and that'll seem to help but like we all know that can come with side effects long term.

I'm a 27 year old male who played baseball in college and was 5'10 185 lbs, went down to 128 in 2014 prior to my emergency colectomy, and am now 153 lbs. id really like to exercise routinely and maybe gain some muscle back without hurting myself in the process.

Thanks Jeane. I saw a new doctor today and he recommended maybe I try Rective (nitroglycerin cream) instead of nifedipine but apparently it's really expensive and can cause headaches. He also said an internal sphincterotomy may help but I wes to meet with the surgeon for an exam under anesthesia to see whether that might work. Hot bath soaks help me as well. It sounds like the root of our problems are different but medical treatment is the same. 

They've never mentioned any stricture to me. Apparently I have an anal fissure that can't heal because my internal sphincter is spasming and so they can slightly cut the internal sphincter so it relaxes tonicity he said he's never referred anyone for this and it could be tricky so he's going to have the colorectal surgeons perform an exam under anesthesia to take a look and see if it's a possibility. I probably won't buy the Rective then since I have viral cardiomyopathy with secondary acute heart failure (which I've recovered from) randomly last year and I have a resting HR of 90 as it is. Thanks for the heads up before I spent the money.
