Hello,
I have had my jpouch since 2001 and am recovering from my first bad case of pouchitis. I tried canasa supps, anusol supps and although they used to work, did not. Dr. Shen called in an Rx for Tinidazole and I feel better but now I have a stricture. I know this because I had one prior to my colectomy in 2000. I live in MD and although I would prefer to see Dr. Shen I don't know if taking care of a stricture requires multiple visits and therefore multiple plane trips to Cleveland. If so I need to find a gastro/pouch specialist nearby either in Metro DC or Baltimore. I posted a request for recommendations for people in Metro DC/Baltimore with expertise in pouch issues and have not received any responses.

I'd like to hear from people who have had strictures and how the issue was resolved.

Hi. Sorry to hear you are not feeling well.
Where is your stricture at? Top of jpouch or bottom? Many folks on here have had dilations on the bottom stricture in the drs office, sometimes a finger dilation. If it is at the top,at the inlet to the pouch, my daughter had it there and had balloon dilations through a scope under anesthesia, also without (yikes! not again)

There are a number of ways to treat strictures depending on the location and severity.

If close to the rectum some people are able to do self dilations at home. Others go on to have this done by a doctor. There are also balloon dilations that can be done. Others find that a liquid or more soft diet eases the pain and helps things pass through the digestive system easier. In more serious cases 2 types of surgery can be done. One called a strictureplasty and another called a resection. In the case of a resection they would have to remove the area of bowel where the narrow portion is located. With a strictureplasty you do not lose any bowel.
Thank you, both for your replies. I just booked a flight to Cleveland. They are going to squeeze me in (something about that phrase doesn't feel right...Smiler ). I can't imagine dilating it myself. It is so painful and feels like I am trying to poop a sea urchin. ouch! I contacted someone Dr. Shen recommended from DC area who is on the faculty at GW Med school and he couldn't see me until Jan 2! I have been on a liquid diet tonight because I am freaking out thinking about the ultimate destination of everything I eat and how painful that is.
