If the BMs are coming in rapid fire bursts after meals, fiber isn't going to do anything. Bursts of BMs after meals means one thing, a spasmodic pouch/excessive motility. It's very common post-takedown. Completely treatable but with an anti-spasmodics like bentyl or levsin 45 minutes before meals. I actually prefer donnatal as it had less side effects.

You can take Dietary fiber but it's not going to solve a motility issue.

You don’t say what quantity is coming out per bathroom visit, but I’d be surprised if your pouch is filling up 10 times over a few hours, unless its filling up with gas. Assuming you’re not producing 10 pouches full of “applesauce” in that amount of time there are three possibilities that come to mind that make you want to empty a not-full pouch: 1) you’re producing too much gas (this would be fairly obvious), 2) your pouch is irritated, creating urgency when a small quantity of stool collects, or 3) you are overreacting to a sensation that you could, perhaps, choose to ignore. Assuming you’re not very gassy the most likely situation is #2 (no pun intended). The most common cause of that is pouchitis. Have you had pouchitis before? Have you tried a course of Cipro or Flagyl?

@readann nope not yet, although to be fair I'm pretty new to the pouch. My takedown was Feb 26. And its not always. But I did citracel yesterday and I've went 4 times this morning, not fully empty. Its frustrating. I will let you know if I find a good mix. I dont take immodium or anything and think I do better when I just stay off the meds like that. I was very cautious for awhile about anything I ate and couldn't gain weight. I've opened up my diet and finally have gained about 7 lbs. Would like to see 10 more. I dont eat raw veggies or salad yet but I think I do better too when I eat a variety of foods. Its definitely a trial and error process.

Also, to Scott's point on pouchitis. I went to Dr recently for uti. Dr put me on bactrim. Two days later called me and said they needed to switch my antibiotic because they got culture back so stop bactrim and take the new antibiotic. However, as soon as I am done with the new one for the uti I am taking the bactrim because my pouch was terrific those few days I was on it. So mild pouchitis, very possible. Flagyl doesn't sit well with me but I've done well on Cipro.

@Scott F Thanks for the reply! For the most part, it is a little quantity. It's about 1pm here and I've gone nine times. Two were slightly more in quantity (right when I woke up, I don't go in the night), and the rest are about 1/4 cup or less. I meant to mention in the original post i have an ileorectal anastomosis (my ileum is attached to my rectum). Unfortunately, there is very little support anywhere for my attachment and my surgeon said some things work similarly to a j-pouch, so I was seeking advice here. So, it wouldn't be pouchitis. Sometimes I do try to ignore it but it builds up and is hard to focus and my abdomen gets a bit painful.

@Kmiller Thank you for replying. It is very hard to figure out the dietary stuff. For some context, see my message above to Scott F regarding my different plumbing. But I found the low residue diet made me feel more "constipated" (the stools were skinny tiny pieces and hard to pass). Eating some fruits and vegetables helps things move through, but it is too much. Trying to find that balance is hard for sure. Are there any foods that are an absolute "no" or "yes" for you?

 

@readann nothing so far thats a no, but I haven't tried too many veggies yet. Working to expand that. I really want a salad but trying to decide which day to ruin if that doesn't go well 😀. So this morning, went many times just a little. Hadn't eaten yet, so ate a chicken lunch meat sandwich and then was able to fully empty. I think sometimes the discomfort is gas causing pressure too. I've been fortunate with food that I don't get butt burn very often.  I stick to water, coffee once a day. Occasionally a soda. I think initially I was scared to try new foods and now I eat much more variety and find i do ok. But the frequency needs to go down. Very rarely runny stools, its more like you've described with consistency.
