I know this has probably been beaten to death, but I am desperate at this point. I have severe skin breakdown in my groin area and up my butt crack. I've been trying everything I can think of. I have tried Calmoseptine, Boadreaux's, Weleda diaper cream, Vitamin A&D ointment, Bee Magic, Triple Antibiotic ointment with pain reliever. I just don't know what to do anymore. The pain is excruciating and it's difficult to sit for any length of time. I'm almost in tears every time I do sit down. Unfortunately with my current position I'm sitting down for very extended periods of time up to 12-14 hours a day. I just tried a sitz bath for the first time tonight and my wife whipped up a mix of coconut oil & tea tree oil to see if that helps. I'm hoping for relief soon, but I've been dealing this for over a month and it's definitely wearing on me. Any help is definitely appreciated. Have a great day.

Hello, Azmedic.  I'm going to try to attach a link to a health company called Medline. They make a product called Marathon Liquid Skin Protectant. It is specifically for damaged stoma skin, urine or stool skin contact, bedsores, and the worst of skin damage. it is a fairly new product.

My nurse tried on me when I had my stoma. My stoma was retracted, causing me agony when my skin broke down and was raw and bleeding. I wept for weeks. The only thing that gave me relief and healed my skin was this product. It's expensive, around $5 for a tiny vial of purple liquid. There are 10 vials in a box. My insurance paid for it. Each vial is used only once; you can't save it for later because it dries very quickly. You can also get it from Amazon.com and I just checked for you, it's on sale now for $55 for a box of 10 vials. You carefully break open a vial and inside is a sponge on top of the liquid. Dab on the purple liquid directly in the wound. It will quickly form a crust, within a minute, protecting your skin underneath and allow it to heal. It does not sting. I remember it gave me instant relief, and the first time in five weeks of suffering every time acid stool poured out of my stoma I didn't feel I was on fire. And I stopped crying. Then reversal at 10 weeks. Now for j pouch issues I use  Zincofax diaper cream. 

I hope you are able to try it, or find it cheaper in a drugstore. I can tell you it is like a miracle liquid. Since you have an extensive area of injury, you could try one vial on the worst, most sensitive area and see if it works. I truly hope you get relief. Please let me know.

Are you putting the cream on before and after you use the washroom? I know it's an extra step but I apply cream before and after I use the washroom. Also, consider just using water to clean the area, as toilet paper can be very abraisive. As others have said, pat and don't wipe. Also consider looking into what detergent you use. If the area is already irritated, soft breathable underwear washed in a chemical free detergent may help. I hope you feel better soon

I had this problem as well and fully understand the agony that you are in.  It's awful.  It took me a long time to figure out how to deal with it.  After several doctors with a variety of specialities, I was sent for skin allergy testing and came back positive for something called Balsam of Peru, including several other things.  While I have many health issues, allergies were never on my radar before and was all a big surprise to me.

The main irritants are tomatoes, citrus and a variety of spices but the list is long.  Anyway, I eliminated tomatoes and citrus from my diet, as well as tackled my other allergies, and I'm happy to report my perianal skin is much much better (and the rest of my body too!).  

Also, my dermatologist suggested that I apply vaseline to act as a barrier before and after each bowel movement and that has helped as well.  The beauty of vaseline is that is has almost nothing in it.  All of the other products I tried ultimately contained things to which I was allergic.

So sorry you are in pain! Here are two suggestions:

1 - Call your local hospital and ask to make an appointment with the Wound Care/Ostomy nurse. (Or go back to your surgeon's office and use the ostomy nurse there if they have one.) I went to the local hospital because my surgery was out of town and I felt I had no one to help me with my skin. The ostomy nurse was great. Took a look, told me it wasn't as bad as I thought, gave me some really good tips. For skin break down, you could benefit from a professional skin care nurse to help you, I think. 

2. I bought myself a special cushion and it saved me when I couldn't sit comfortably. Look up Tush Cush. I got a cheaper version of it.

Oh, and here's another vote for Ilex paste. It's good on open skin. The trick is to use a little bit in a thin layer. Then it dries like a second skin.

Thanks y'all. This is the first time I've asked for advice and y'all have been so supportive. After my wife actually getting a good look at it she told me it looked like a fungal infection......������ So, after talking with my PCP and GI they agreed so I am starting with a topical cream for now and see how that goes. If it don't work then I'll end up taking an oral treatment. Some of the pain and burning have let up, so we shall see. Thanks again for the support and y'all have yourselves a Happy Thanksgiving!!!
