What does your urologist/oncologist tell you? Having a j-pouch is secondary. Prevention of future spread should be the priority. Your Gleason score is a sum and it can mean different things. Other factors, like your age factor in too. 

If you are most concerned about your j-pouch, hormone therapy could be a good start. But, you need all options laid out for you. Prostate removal is not always the first or best option.
