I have had my jpouch for 11 years without a single issue (had UC prior with no sign of crohns). I had my thyroid removed 4 weeks ago due to papillary thyroid cancer and started replacement thyroid hormone. Since the surgery, I have had periods of difficulty emptying my pouch...feels like it stops short and there is trapped gas, and a feeling like it is out of sync with my bladder and emptying my bladder takes some effort too. The first time it happened, it resolved in about 4-5 days and I assumed it was done, until now. 

I did notice that both of these periods of time when my pouch was being obnoxious were periods when I was out of my normal routine. Once during surgical recovery at home and now after a vacation. Both times I had not been having my usual morning coffee. Any chance that alone could be the issue? Could thyroid meds cause pouch issues? I'm not sure what to think.

My gastroenterologist recommended Metamucil because he calls it a bowel regulator.  If you're constipated, it helps move things along.  If you have diarrhea, it bulks things up. Also, if I'm having a hard time passing stool I eat an apple....skin and all.  It works for me!   I swear by either product (apples/Metamucil) when I'm having changes in my typical elimination experiences!  Best wishes! 

Thank you all so much for your advice. I gave up on Thursday and called my surgeon, who thinks I may have pouchitis or cuffitis. She put me on Cipro and I have an appt in two weeks. 

After one dose of Cipro, I could fully empty my bladder again in one sitting and my abdominal pain and gas pressure was gone. Now on the third day I can empty my pouch much better, and sometimes fully. I think we're on the right track.
