I am going to see Dr. Kiran @ the CC and have a k-pouch put in. This will be my 4th surgery performed by Dr. Kiran since 2007. I have heard so many good comments about this procedure that this si the way I want to go. How are you or anyone else out there feeling after their surgery. Any info would be greatly appreciated.

I never did say thanks for the info, so.......THANK YOU!!!!!!! Dr Cohen here in Toronto was amazing, aside from post op pain, the pouch and nipple valve are working perfectly! It's been not even 6 weeks, and already I'm down to 3-4 emptyings a day, AND, for the first time in the past 23 years with my old J pouch, I can sleep right through the night without having to get up and go to the bathroom, that is a miracle in itself!! I only pray the post op pain and weakness goes away, then I'll be perfect!

Keep up the good work Eric and don't go too fast..Take your time healing...the slower you go now, the better things will be later one...no working out, no abs, no carrying heavy weight (grocery shopping is OUT!), no climbing stairs or bending/stretching/pulling or pushing.

I must have lived on Mars , as I always felt like an alien and
did not know that these Specialists existed in Southern CA.
Reason being, I did not have much of a problem all these years
with my K- Pouch. I have always considered myself very lucky
to have had this OP in 1980 because it was in the early stages.
I was referred to a Dr. Alex Gerber in the Alhambra Hospital, who
had learned this technic in Sweden. He retired soon after and has
long passed away. Except for enlarging the stoma and a time of
Anemia, oh and retrieving a Plum pit, I had no problems and
acted like nothing ever happened because I was able to do all
the sports that I was used to doing. 5 months after the OP you
could find me on the mountains skiing.
When I finally did have some problems years later, pouchitis, none
of the doctors I knew were around. I had heard though, if it does
happen, get Flagyl. Ever since, I was only treated by a GI doctor
and since I am on Scan Health Insurance with a very good doctor
but nearly no knowledge of the K-Pouch. I feel like I am teaching
him because I have been living with this for 32 years.
After having low abdominal pain for a month, went to the doc.
had a CT Scan and finally after 2 1/2 mos. he found the courage
to look into the pouch with my encouragement. Made a big deal
about it and probably costs lots of money in the hospital, he saw
pieces that looked like stones. I told him about the plum pit and
he got a bigger scope and basket and fished out 3 stone like
creatures but by that time I was pretty sore. I had chosen no
sedation because I wanted to be alert with this fishing expedition.
He stopped and everyone was at WOW. Well, there are two more
Stones to fish for. After the soreness calmed down, I felt somewhat better. I finally had some of my energy back that I
had missed out on for almost 3 mos. and living on Ensure and ground up food. Needless to say, I lost a few pounds and I don't
need to lose weight, I only weigh barely 100 pounds.
MY QUESTIONS IS. DO YOU KNOW OF ANYONE THAT HAS HAD
THIS PROBLEM WITH THE STONES? I do hope that I will be well
again but I will check into one of the doctors near me that you
had on your list. Any recommendation?
I WOULD SURE FEEL A LOT BETTER WITH A DOCTOR THAT HAS
EXPERIENCE WITH K-POUCHES.

Hi!! wow, we both had Dr Gerber in 1980! I had no idea or have met anyone else! His assoc's took over and they did my valve revisions but they are retired. I need to check out their offices to see if anyone is doing them.

In the meantime, we found Dr Launer out of Scripps in La Jolla who trained with Dr Kock and Dr Gerber. He unfortunately passed away last year. But, his assoc Dr Worsey is very capable and worked hand in hand with Dr Launer and independently doing KPs.

I haven't heard of stones - I know you can have very old undissolved medication that looks like stones. Did they send them to the lab? if, so what type of stones were they? Also, you may want to start a new discussion with the headline "pouch stones" to see if any members respond....

I'd set up a consult with Dr Worsey. He can also do a scope right there and then in his office. Yes, no sedation..hurts like hell for a few minutes but no biggie as you well know.

How wonderful you found us. I also had no idea until I found this wonderful forum in 2008! I'm PM you...let's get together! We have a small group of KPers that try to meet for lunch annually!
I am new to this site and appreciate all the feedback and input.

I have been researching the k-pouch and find it the best option given all my problems with my j-pouch and current ileostomy (as well as a fistula). I too see Dr. Kiran (as well as Dr. Shen) at the Cleveland Clinic. However, my last appointment, I was told I have to wait for the K-pouch surgery (6 months to 3 years). This is truly frustrating since I have nothing but pain (cuffitis) and problems with my ileostomy. Is anyone else familiar with this 'waiting period'? Any insight as to why?

Hi Kara, I too went to the Cleveland Clinic. I had Dr. David Deetz as my surgeon. I went in to see him in October 2011 and there was only 1 Friday that he couldn't perform my surgery. I could have had it anytime I wanted but with my husbands schedule, we choose to have it in May 2012. Had surgery May 4, went home the following Saturday May 12. Dr. Deetz was awesome, had no complications and to date have had only 1 case of pouchitis. grandma64
From what I have heard, the wait period is not due to scheduling issues, but because of what they need from the patient in regard to physical readiness or to rule out contraindications (like Crohn's). I imagine it varies case by case. Kara, what have your doctors told you about the reasoning? If they have not told you, you need to find out.

Hi Kara, the surgeons at the CC are the best for kpouches. If they advised you to wait, they must have surgical rationale. Why don't you give their office a call to clarify?

It could be a number of reasons, a. to have your digestive track settle down, b. to gain weight, c. to gain strenght, etc.

Very sorry to hear about your chronic pain, they should have addressed this with you at your appt while advising you to wait on the KP. I'd ask about that, too.

Dr. David Sung has retired. A great loss as he was an incredible surgeon. When I learned that he was going to retire I freaked out and it was for a good reason, as this list shows. He did say he tried to train younger doctors but couldn't find anyone who was really interested. That is the scariest thing about my health.

I just wanted to thank you for keeping this list. It was instrumental in my decision to move to New York to see Dr. Kiran. He literally saved my life. He is honestly the most intelligent and masterful doctor I have ever encountered (and I've see hundreds). He trained under the great Dr. Fazio at Cleveland Clinic and is now chief at Columbia. I cannot recommend him enough for any IBD related surgery. In any case, thank you for playing the role you did in all this.

No Neil Mortensen has not retired from private practice, only public practice. I just heard from him today

I'm a professional golfer and good tennis player I found that Brooke ileo just didn't allow me to play same standard as before. Dr David Dietz has not been at Cleveland Clinic for a few years now. He is at University Hospital just down the street from Cleve Clinic

I just can’t stress enough how wonderful Dr. Kiran really is. This doctor is literally the ONLY ONE who stepped up to the plate when no other surgeon in the country was willing to take on my case. He is the reason where my health and overall well-being is at today. I’m very fortunate to not only have found such a gifted and talented doctor but also a human being who looks past beyond medical reports/diagnosis, to show true concern and comfort.

I called most of the Drs listed yesterday.

 

Dr Shore stopped doing them several years ago. The website is defunct.

Dr Beck retired in December.

Dr Remzi is in New York not at CC Ohio.

Dr Fazio passed away, I am sure you know this.

Dr Wexner does not do K pouches and neither do any Drs at Cleveland Clinic Weston FL. I just had my j pouch removed there. They refer to Ohio.

Janicem   Great List   I have had a K Pouch since 1985 Performed by Dr Randall Bollinger at Duke. Unfortunately he retired. I live in Southern WV and am a retired PA. Looks like there may be a MD in Roanoke Va  Dr Fogel. Thank you so much for all this work. With no malice intended, most physicians acquire this distant stare when you mention a Kock Pouch. Fortunately I have a PCP who listens to me when it comes to the pouch

Dr. David Dietz is at University Hospital in Cleveland. His website notes he is accepting new patients.  216-844-7874  

He did my surgery in 2015 and I was extremely pleased with the results. Just  to add, he removed scar tissue on my intestines. This resulted in going from as many as three painful (and dangerous blockages) a week to none since then.  Obviously I highly recommend him.

J-pouch surgery is an option for some but not all patients. The surgeon who removed my colon cut too much tissue so a J-pouch was not an option. I am very, very happy with my K-pouch, though.

There are maybe five surgeons in the world who are highly trained and skilled enough to create continent ostomies like K-pouches and BCIRs. WARNING: there are a LOT of unscrupulous surgeons who CLAIM that they know how to do it, or that they can do repairs, and instead damage the continent ostomy or even REMOVE it. PLEASE be careful when choosing a surgeon. Check these boards for RECENT recommendations from patients who have actually been treated by a surgeon and endorse his/her surgical skills and post-surgical care.

There are maybe five surgeons in the world who are highly trained and skilled enough to create continent ostomies like K-pouches and BCIRs. WARNING: there are a LOT of unscrupulous surgeons who CLAIM that they know how to do it, or that they can do repairs, and instead damage the continent ostomy or even REMOVE it. PLEASE be careful when choosing a surgeon. Check these boards for RECENT recommendations from patients who have actually been treated by a surgeon and endorse his/her surgical skills and post-surgical care.

Were you mad originally at the surgeon that cut too much tissue off???

The surgeon who cut off too much tissue knew I had a prime malpractice lawsuit on my hands... so (without mentioning 'lawsuit') he offered to send me to his surgical mentor — who happened to be the head of colorectal surgery at the Cleveland Clinic and the top K-pouch surgeon in the world, Victor Fazio. I would never have found out about K-pouches if this idiot surgeon hadn't been trying to save himself from a lawsuit! And I could not have gotten luckier -- the late Dr Fazio is a legend who other colorectal surgeons talk about with reverence and awe. So a disaster turned into a miracle. Yes, I was angry but I was also choosing my battles carefully. I'd been so sick --down to 75 pounds at one point  -- and I had to focus on the future.

LOL, you are funny! I love it! I also love your optimism! You must be a really happy person! We all can learn a lot from you. You must be stress-free too.

I hear K-pouches are a great alternative to a ileostomy. Its probably hard to find a gastro that is experienced with scoping it for check-ups.

How often do you have to empty out your K-pouch??? Every 3-4 hours right?

And how often do you have to change catheters???

It must be hard to clean the catheters and taking it with you wherever you go too. You guys probably keep the catheter in a bag to take it with you when you go out.

YES, I HIGHLY HIGHLY recommend the K-pouch! There was a whole year between the removal of my colon and my K-pouch surgery when I wore an ostomy bag so I know what that entails. Frankly, I hated wearing the bags and emptying them and all that went with it. The K-pouch has been one of the greatest miracles of my life and I post here and recommend my surgeon so that people know they have options that most surgeons won't even mention because they don't do the surgery themselves.

How often I empty the pouch depends entirely on what and when I eat and drink, and if I'm having a busy day and don't want to have to empty her frequently, I can adjust what I put into her -- less gassy foods, less bulky salads.

Catheters will be covered by your medical insurance and you just rinse them out in the sink when you're done intubating. They're made of flexible silicone, and they stay pretty clean for weeks but you can change them as often as you want. I keep one in a baggie in my handbag with a packet of lubricant so I can empty her 'on the road.'

Knowing what I know  with 18 years of experience I would definitely get this surgery again even if I lived a few hours away from the doctor I needed to see if I had any problems. I traveled to Cleveland twice to get this surgery and still think it was one of the BEST decisions I ever made.

I have annual checkups with Dr Kiran in NYC, where I live. (Dr Kiran at New York-Presbyterian/Columbia University Hospital (212) 342-1155) I had no problems for years and years so I don't know that you really need to have an annual checkup -- but knowing what I know  with 18 years of experience I would definitely get this surgery again even if I lived a few hours away from the doctor I needed to see if I had any problems. I believe its recommended for us "POUCH" people to get scoped every 1-3 years.

I love how you call your K-pouch "HER", I do that too with my J-pouch. Have you named your K-pouch???

I named my J-pouch, "The Emerald City Of OZ". I am a huge WIzard of OZ fan, I read all 15 books. Emerald City is a magical city and so is my pouch. I call her EMMIE for short
