Jpouch and fistulotomy

Hello all… been a long time lurker but I’m in need of some advice… had my pouch since 2010… relatively smooth sailing aside from annual pouchitis troubles… fast forward to mid 2019…

I had an anal abscess develop which was drained and resulted in a short hospital stay. The resulting wound never healed and continued to drain. MRI confirmed fistula. 3 procedures later I now have a seton since summer 2021. surgeon now believes the fistula is mostly superficial with little to no muscle interaction and wants to perform fistulotomy. I’m terrified of how to live “the pouch life” with a anal wound like that…

has anyone with a jpouch gone through a fistulotomy before and can offer some advice?
