Ok I know I have posted and read posts on this before, but now I am beyond desperate. I got my jpouch in April 2018 and have experienced leakage ever since. On 2 occasions I have had pouchitis 2 x and know the symptoms. There is absolutely no way I should have leakage everyday all day everyday night all night. It is really taking from my quality of life. I am afraid to eat to much. I don’t eat after 7Pm although I do drink because I am so thirsty after bowel movements. I have taken Lomotil, Imodium all to no avail. I’ve tried different fibers with no results. I am so upset over this as I don’t know what to think or do. There is no way I will go back to the bag. Can you have I going pouchitis without all the other symptoms? I can’t figure this out for the life of me. I will be making an appointment for my GI but I’m the meantime does anyone have any ideas and if you experienced this what did you do. I’ve also tried kegels my PT said my sphincter seems to be strong however while trying to hold it I still leak. I wished I could say it’s small leaks but it’s big leaks that have me worried all the time . Since my jpouch I have to wear a pad all of the time. Please help I am desperate. Thank you!

I think you may want to put a call in to your doctor. In the meantime:

Acute pouchitis is sometimes treated aggressively with both Cipro and Flagyl. Not all doctors do this, probably fewer than half, but in any case the goal is to wipe out the episode and be done with it in 10-14 days. Chronic pouchitis is treated a bit differently, and the goal is to find the minimum dose of medication necessary to control the condition (some try to control the symptoms, others try to quiet down all of the inflammation, too). You haven’t described the time line, but if Cipro alone makes you feel better then you’re probably better off without adding the Flagyl.

You can have pouchitis without regular symptoms, since you have had pouchitis before it's definitely possible it's back. Going to see your GI is probably for the best.

I don't present with typical symptoms, I can still manage 6bms per day, I just can't sleep, sometimes my BMs do increase.
