Hi friends.  Just wondering if anyone else has any experience with Internal intussusception of rectum? Visited my surgeon a few months ago and this is the diagnosis.  She prescribed pelvic floor therapy and indicated that surgery to "tack up" the pouch could be helpful.  However she refuses to do surgery until I reach a "normal" (not overweight) BMI.  I'm a 25 year j-poucher, with somewhat mixed results.  Just had a normal looking pouch-o-gram, so the pouch is healthy.  So, I'm not sure how serious this diagnosis is.  Anyone ever heard of this, or have been treated for this? TIA.

These are not common! Part of the gut makes a circumferential inward U-turn, and the next section of gut slides inside. It’s a big deal, and causes blockages and worse. Since almost all of us have had our rectums removed (the rectum is not the anus), that’s not the affected part, but ileum could potentially protrude through the pouch inlet.

Thanks Scott for the information and the diagram! That's more information than I got from my surgeon.  I'm not in an emergency situation, thank goodness.  And, my surgeon won't consider surgery until I lose about 50+ lbs! So, it'll be a while before I will be considered a candidate for surgery.  I was hoping someone might have experience with this.  My symptoms are having to manually "push" the pelvic floor back into place after I potty.  (and yes, I have a BM every time I use the potty - about 10-12 per day).  I also have hemorrhoids that are part of the issue too.  Oh the joy 😅

This sounds more like a full-thickness prolapse than intussusception.

A full-thickness J-pouch prolapse occurs when a circumferential part of the pouch protrudes toward or through the anus, inside the pouch portion attached to the rectal cuff. Prolapses commonly interfere with pouch emptying. If it isn't obvious it can generally be diagnosed with a defecogram (a contrast study that images the pouch while pooping). Often prolapses reduce themselves when straining stops, or things can be pushed back manually. If it needs to be treated it's generally handled with surgery to stabilize the pouch.

I’d suggest consulting another surgeon for another opinion, unless you have reason to believe that the weight loss will be pretty straightforward. While it’s true that many surgeries are easier and safer when body fat is low (or lower), it’s also true that surgery tends to be needed by people that exist, rather than imaginary future people who weigh less.
