I've been dealing with a couple of painful burning hemorrhoids for the past three weeks. My Doctor has me on a night time enema medication that seems to be helping the hemorrhoids. One issue i'm noticing lately is when I'm standing or walking around, I can't seem to go for longer than a few minutes with having some leaking. And the longer i stand the more leaking occurs. I'm at a loss because I've never had leaking problems before. WIth these hemorrhoids I haven't been able to really do keegles or anything but is it possible the hemorrhoids are actually keeping me from having a full closure and thus allowing stool to leak? It's really hindering my ability to work and do anything where i have to stand for a period of time. Thoughts and/or suggestions would be greatly appreciated.

It's Mesalamine. I was on the hydrocortisone suppositories first but they didn't seem to be helping. This seems to be helping but in the last couple of weeks I'm having a hard time holding it and have had many more accidents. My appetite has been getting worse, and I'm losing weight. I've struggled with weight gain since my last pouchitis bout and then these hemorrhoids have just made things so much worse.
Mesalamine is not a hemorrhoid treatment. It is for UC, so this confuses me, unless you have cuffitis that is making your hemorrhoids worse from the nearby inflammation. How about trying mesalamine suppositories (Canasa)? Easier to retain than the enemas. Another option is rectal cortisone foam (Cortifoam). It does sound like you have pouchitis too. Have you tried a course of antibiotics?

One reason they might prescribe the enema over the suppository is cost. There are generic mesalamine enemas, but not for the suppository. Your plan may require failure of cheaper options before expensive ones are covered. Also, sometimes those suppositories can cause local irritation (they are pretty large).
