Hi all,

I’ve had my j-pouch since 1997.  While it hasn’t been great, I’ve had a Crohn’s diagnosis since 2000, it’s been tolerable.  In the last four months I’ve been dealing with a fissure that is quite painful.  (This is the first time I’ve had one.)  I’ve tried lots of meds, ointments, rectal rockets, budesonide suppositories and antibiotics, but nothing has really worked.  

Does anyone have any suggestions?

@amtrpt It’s hard to know from your description what you’ve actually already tried, and fissures can take a lot of patience. I assume you’re using warm sitz baths very regularly. In any case, in addition to the nitroglycerin JHendrix mentioned, some folks have success with nifedipine or diltiazem compounded into an ointment. If all else fails Botox is sometimes used to temporarily reduce anal sphincter pressure, which can give the fissure a chance to heal. Good luck!
