I have a j pouch but two years ago got ischemic bowl and went back on the bag and now I have a fistula that leaks from my abdomen , any help would be great .

I have a perianal fistula... Not the same thing. No real advice, except hang in there.

What do you do for it now?

This sounds like it was a complication of your surgery. Has your surgeon suggested resection of this? It probably will not heal on its own after this long. But, perhaps your history precludes further surgery as being too risky. Can you give us more info? Most of us had colectomy due to IBD or FAP. Ischemic bowel is different, so your risk profile is different.
