i can relate my situation, as it was similar to yours. I had takedown 1 year ago. Several months later, in summer 2021 I started having what I thought was pouchitis symptoms.  Surgeon said impossible, but GI said possible but unknown until pouchoscopy could be performed. I had similar calprotectin numbers. We tried cipro for two 2 week intervals. I thought it helped.

As it turned out pouchoscopy revealed cuffitis  and other tests revealed SIBO.

others will post here with more experience with pouchitis treatment.   my take is that a short course of antibiotics seems to treat multiple symptoms and you should feel better.
