Went to the doc today for check up. Said he could not see anything because there was blood present. Been having some intermittent rectal bleeding with stools. Have been using Proctofoam for about 4 years twice a day to stave off cuffitis.  He told me to stop the Proctofoam because chronic use could cause bleeding. Starting me on Rowasa enemas to see what they will do. I eat a lot of chocolate daily (have been trying to cut down lately).Was wondering if that would also cause some of the bleeding with the Proctofoam. He also mentioned maybe surgery in the future but we would have to see what happened with the enemas. Any ideas?

I don't think the chocolate is doing anything to exacerbate your cuffitis. I have some concern about stopping the proctofoam abruptly after four years of daily use. It might be fine, but if you start to have strange symptoms (not GI in particular) don't delay in calling the doctor that told you to stop. If you were bleeding enough to prevent a proper exam (pouchoscopy?), then trying something new makes sense to me. Getting the cuffitis under control without surgery is well worth trying to do. You may want to try Canasa suppositories which are non-steroidal unlike Proctofoam which might mess up with the blood vessels it is absorbed in over time. Your doctor may be raising the possibility of mucosectomy with pouch advancement. It is effective for chronic cuffitis but obviously is not a surgery you want to have unless necessary.
