I have had multiple fissures for 6 + months now. I kept thinking it would go away. But the pain and spasming and bleeding got so bad I ended up in the ER. I have since seen my GI almost 2 mo post ER visit--first exam--too painful in ER. My white count was somewhat high, along with other high or low labs indicating inflammation or infection or Crohns flare.

GI thinks possible Cuffitis and upon very painful exam, rectal canal is extremely tight. I'm having a Pouchoscopy Wed. She says it can be common after an Ileoanal Anastomosis. I am almost 20 years post surgery having a history of obstructions, as well. She is prescribing a foam steroid and ordered the pouchoscopy STAT to the top of the list. Clinic stated she was very concerned.

I also had dysplasia of colon in the upper 2/3 prior to J-Pouch surgery, which concerns me as my NP stated the risk for cancer is there as well. Most of my father's side succumbed to colon cancer early in life, including my dad.

I'm seeing other people's stories about cuffitis and pouch excisions as a result. I am not sure how far the fissures go up inside the rectal canal/pouch. I was dx'd with "Atypical Crohns" (which I do not understand) post surgery instead of UC, which they originally believed it to be. I certainly don't want to lose the pouch for an ileostomy!

If the fissures make examination painful, maybe you could ask for mild sedation for your pouchoscopy tomorrow. You don't want to be in pain during an exam since that might make you tense up, especially when the doctor needs to manuipulate the camera around inside for a 360 view. See if you can call your doctor today and ask for sedation. It might be a pill or an IV line, at least they will be prepared for you. You will need someone to accompany you home after sedation.  I get nervous and tense up and that's not good since I want my surgeon to have a complete look inside and take biopsies. My j pouch was the result of cancer so I'd rather be asleep and relaxed so he can be thorough, and not rush to the nearest exit.
