I have a bidet and it has really helped a lot.  They have less expensive ones on Amazon.  Also, although I have been doing well with Calmoseptine, I have heard that Ilex paste works well.  You can get that on Amazon as well. You have to be vigilant about using it after every BM.  You might want to give a call to your doctor as well.  Maybe they can prescribe something that works better than OTC ointments.  I am very new to this as well, one month since takedown, getting used to all this stuff can be very frustrating.  Good luck!

Hello, Clark. Do you have a sitz bath? You can have it delivered from Amazon. Approximately $15 to purchase. It is a special basin that you place on top of your toilet seat (seat up) and fill with water as warm as you can tolerate. Overflow water will drain out the opening at the back of basin. The very warm water brings blood to the perianal area to help ease the pain, especially pain from fissures, which are caused by straining, hard stools, frequent stools. Perhaps use a few times a day for soothing comfort. A travel bidet bottle can also be bought from Amazon, $12 or so. Use the bidet water bottle after every BM, no matter how much, or how often, just so you keep the stool and acid off your skin. Don't use dry toilet paper. Use unscented baby wipes, or even dampened paper towels (don't flush these; put them in the wastebasket so you don't plug up the toilet pipes). My doctor told me to use unscented soap such as Ivory or unscented Dove on the area. How long since your takedown? I hope this helps you.

I've been there.  I know what your feeling.  Something that doesn't go away day or night. It gets unbearable and will beat the strongest down to pulp.  Never posted your way.  But I'll be damned if I wasn't saying it in the bathroom!  No painkiller can take it away. 

I hope you can solve you issue.  Lots of good advice here. 

I had a nasty fissure 6 months ago. This is what helped me;

1. Pawpaw ointment (this is the best ointment i tried about 10 different varieties)

2. Nifidepine , this is much better than the other one that gives you headaches (i had to get it made up at the chemist)

3. Stop eating veges as they cause to much gas. Just go with protein (chicken, fish) and rice.
