I could really use your help as I navigate through some issues that I am having. I had my J-Pouch Surgery at CC in December.  Recovery has been slow but trying to be kind to my body as reading others stories, it could take a year or more for things to "normalize".  

Since surgery in December, a little bit of poop has been coming out of my vagina. I thought as things heal, so would this.  

Last week, a little became quite a bit - I am needing a thick pad to make it through the day.  I got a CT scan to confirm it was a fistula and they also did a scope and stated that my cuff is really inflamed which could have caused the fistula.  Here is what I am trying for the cuffitis and fistula:

1. Canasa suppository
2. Cortifoam
3. Estrogen cream

If this doesn't work, they talked about getting my loop ileostomy back and doing a pouch advancement.  From what I understand is that the cuff would be removed and the pouch would be advanced over the fistula.

Here are my questions:

1. Does the topical stuff really work?
2. Has anyone had these issues and did the pouch advancement - I am worried that removing the cuff would make me more incontinent.
3. What if I got the inflammation under control in the cuff and just had the fistula fixed?  

Any advice and information would greatly be appreciated.  I know I should be posting on here more than when I just have my own issues.

I am sorry to hear about your troubles. I also had a recto-vaginal fistula and cuffitis. I ended up getting the cuff stripped (which meant getting another ileostomy, revising the j-pouch, getting the fistula fixed and then reconnecting). I am currently recovering from the takedown. So far so good except the fistula opened up again before the last surgery. I have a seton in but not a lot of leakage. I see my surgeon in 3 weeks for follow up and will ask about the plan for the fistula. How are you feeling?
