I had the pouch done 18 years ago in 1 surgery. I don't take anything - no supplements or anything to help with regularity.  I eat and I poo, 5-6 times a day.

I've had these kidney stones growing inside me for the last 10 years or so.  They started off small and passable, but over the last few years, they've gotten big enough that they're not going anywhere.  The last 3 months they've started getting quite painful and its time to have them out.  This is the first time since the pouch that I'm going under the (very small) knife - Percutaneous nephrolithotomy.  The largest is 22 mm, and there are two more that are almost as big, so we decided not to do the Lithotripsy (the procedure where they send a scope up to blast them away) because it would take at least two sessions of it, and likely wouldn't clear things out entirely.  My concerns are with the general anesthesia.  I leak from time to time on a good night (pretty rare, except when it's not), so of course I have concerns.  I guess my ask for advice is around the food / drink rules.  They say no food after midnight, but even if I have no food after 8 PM, I'm still pretty active - normally once overnight, then around 7 AM, and again around lunch time.  Does anyone have any tips or tricks?  Should I make my own rules and just skip dinner entirely and maybe lunch too the day before surgery?  I'm terrified of waking up from surgery in a mess.

I had a similar concern with sedation and anesthesia. I did have a small mess after an upper GI endoscopy. For bigger procedures I mentioned my concern to the nurse and was reassured by them and didn’t find anything to clean up. I’m guessing that they might have cleaned me up, but I was asleep at the time. I have always emptied my pouch immediately before climbing up on the gurney. Eating a light dinner or skipping it might be helpful, and letting the nurse know of the possibility would also be wise.
