I had my j-pouch take down in 2015, following a 10 year battle with UC and since then there is little rest from the battle, all be it a new one! I've tried multiple medications, none of which seem to work for long (if at all).  I currently have loperamide (30mg a day), which is supposed to slow things down, and use codeine and paracetamol for pain, but right now, none of these are working either! I've been prescribed Questran for the last 8 months,   which helped with acid output and ‘butt burn’ and it seemed like it was helping although we currently have supply issues in the UK so can no longer get get this!  Then I had a gallstone attack 4 months and needed my gallbladder removed. Since then, nothing has worked.

I can't seem to lower my output to anything less than 12 times a day (plus at least twice at night), so I eat well but cannot gain weight because the food doesn't stay in me long enough, and 'butt burn' is a constant agony.

I have been smoking cannabis for about 3 years following very 'off the record' discussions with my consultant, as I had no appetite and couldn't sleep, and there was always pain somewhere and prior to my gallbladder removal and it definitely helped with the sleep and appetite but as I’m in the UK, this was all a little naughty as up until recently all forms were illegal.

We can now buy CBD products in the UK, and although it is cleared for medical use it really is in the most extreme cases and diseases (and even then almost impossible to get it signed off by doctors).

Because it's so new here, no one really understands it, and can give me any real information on what's best for Pouchers! There’s a lot about how good it is for UC and CD but not after surgery.

Any advice or information you can share would be really appreciated.

There are many thread on this topic and some are quite vigorously debated.  Rather than re-invent the wheel I would suggest you use the search function.  I will try to post the liveliest of the many threads to get you started:

Fellow UK/English guy in that thread seems like he does not approve of Cannabis.  Me personally, I do not understand scientifically how it would help inflammation and nobody here has been able to explain that to me.

There are many other threads, search under "marijuana" as well as "cannabis"

I’ve spent days reading everything on here, but what I’d like to know is what products/ strengths people have used and how they helped (if at all)!

I started with a cannabis clinic here in Canada. They walked me through everything from dosing to learning what the best way for me to consume the cannabis would be. YOU DONT HAVE TO SMOKE IT! As a matter of fact, its better for your bowels to ingest it (Capsule, or Oil.)

For some great info see:

Take a look through the website as there is lots of great info.

Contact me anytime I would be glad to help in any way.

About 12-18 months ago, I started having generalized but pretty severe (often a 7/8 out of 10) stomach pain. Not reflux (I know that pain). Not gas (I know that too).  Not down in (what's left of) my bowels.  The pain was located in a sort of arc around my  rib cage.  Nothing helped.

A colorectal surgeon friend said the pain could very well be "residual" pain from scar tissue left over after colectomy.  She said there is probably nothing to do, and I just had to live with it.  F that.

On advice from a friend dealing with Rheum Arthritis, I tried 30:1 CBD:THC tincture.  1/2 ml, 2x per day (morning and afternoon).  It actually worked!

I have now tried multiple branded CBD tinctures.  Some work better than others.   THC is actually the key.  You need a trace amount of THC to trigger/activate the CBD (this I've learned from experience and from much reading and discussion).  SO, the CBD tincture must be derived from Cannabis plant (which has THC) and not Hemp plant (which has none).

The 30 parts CBD to 1 part THC ratio is key for me.  The THC is so minimal you do not feel high at all.  You may not need 30:1. I've seen tinctures with 4:1, 5:1, 15:1, 20:1.  As ratios go down, however, I've noticed that the 'highness" goes up.  1:1, for example, is basically smoking a joint.

The tincture is made of "MCT" oil, a highly soluble form of coconut oil.

Hope this helps.  J

Juanaton, have you tried to find a physical therapy that specialises in dealing with scar tissue in the abdomen?  I’ve been seeing someone that has worked wonders for me in various parts of my abdomen, including near my rib cage. By no means has it cured all of my many problems, but it has helped a lot.

I’m in London but I know she trained in Canada.

Also, I will be moving to Chicago soon and plan to look into CBD once there.

I have a lot of scar tissue and it has become “sticky”.  The scar tissue  sticks to my abdominal wall, particularly around the rib cage.

In most people, everything in your abdomen sits in harmony and fluidly moves around. If one has an organ removed, the natural order of things is out of whack. I’ve had four abdominal surgeries related to my J pouch and a hysterectomy. The adhesions stick to things, which creates pulling. The physio attempts  to make things less “sticky”.
