Rectal Pain and Straining

Hello!

I had my takedown in May, 2019 and ever since then have had intense, sharp pain in the lowest part of my rectum.  My dr assures me that the pain will go away but it keeps getting worse.  There is a narrowing of my rectum and the dr said that she stretched it back in June but it has not helped.  The pain (like glass in my butt!!!!) gets worse upon bending over, squatting and during bowel movements.  When I try to have a bowel movement the straining is massive, I'm always straining.

I did two rounds of antibiotics and that seemed to help but as soon as I stopped the symptoms came back.

Any thoughts/suggestions as to what I can do to alleviate the pain and straining would be appreciated.

Also, (and I will be graphic LOL), where do you put the barrier cream?????  Do you actually insert it into the lower part of the rectum/anal canal or just the exterior of the opening?  Sounds silly to ask but these are the things that the dr. never discusses.
