Questionable hemorrhoid

I am.bramd new to this group. 12 days ago I developed a sudden hard grape sized lump at anal opening starting about an inch up. Looks and feels different than hemorrhoids I have had in the past. I have a j-pouch for 2 years now. I have frequent itchy fissures which adds to this new problem. Doc at emerg said probably a hemorrhoid. I think not. Sitz baths and Anusol HC doing nothing. Witch hazel pads do nothing. This is not shrinking at all and the pressure is very uncomfortable. Appreciate any thoughts.
