A full-thickness J-pouch prolapse occurs when a circumferential part of the pouch protrudes toward or through the anus, inside the pouch portion attached to the rectal cuff. Prolapses commonly interfere with pouch emptying. If it isn't obvious it can generally be diagnosed with a defecogram (a contrast study that images the pouch while pooping). Often prolapses reduce themselves when straining stops, or things can be pushed back manually. If it needs to be treated it's generally handled with surgery to stabilize the pouch.
