Another "is it" question and antibiotics

I'm in my late 40s and have had my Jpouch since I was 15. I've mostly been ok with a few bouts of pouchits and gastritis, but it's been a few years. The past couple days I've noticed cramps after eating, and the feeling that I have to go, but then I barely go. I do strain and the end feels irritated when I do.

Question 1 - Does it seem like pouchitis?

I haven't seen my gastro dr for about 8 years when I had gastritis so don't even know if I can just call him up (but he's the dr I saw when i started the UC at 15). Other obvious issue now is covid..last thing I want to do is visit a hospital.

Question 2 - Do Dr's ever prescribe the antibiotic (flagl? It's been awhile so i forget the name) without an exam?
