Anal Pain-Itching

I have been going a lot and all the wiping has become very painful and itchy (when it's not so painful). It's really making it impossible to concentrate and get things done. I've tried vaseline and Desitin, with limited success. I take baths and use warm wash cloths, when I can. However, during the workday, I need some serious relief. Any other suggestions are really appreciated!
