Hi all- just an update from my ongoing leaking issue. I saw Dr. Dietz yesterday and he scoped me again. Appears my valve has slipped more (from 2 centimeters to 7 centimeters?) and I will need surgery to repair it. He believes it slipped because the scar tissue that normally forms around the valve to abdominal wall never really formed, so it allowed the valve to pull away. If the slippage was still around 2 centimeters, I could have controlled the leakage with diet, a night catheter and or a day catheter. But in this case, the slippage was too much. I could have done nothing to prevent this as it happens in about 1/3 of his patients. So I am back in July 8th for a repair; the recovery and follow up will be the same as the K-pouch surgery. At least I know what to expect so that makes it easier. It is just the way it goes for K-pouchers. I am optimistic because I know why I am leaking, it wasn’t something I caused, and I have a good surgeon and good stoma nurses to help me once again. I am not looking forward to the pain and recovery of this surgery (again). I will certainly be more appreciative of having a K-pouch and take better care.  Thanks for all your advice and insight!

i agree it is always more comforting to know what the problem is, however it isn't easy to go through surgery again.  and at that, the same as before!!  wish you could have gotten in right away and get it over with, but i trust this is what worked out for your schedule and cc's also.  i'm surprised to hear the 33% revision rate.  when i previously inquired he said it was about 10% during the first year.  hang in there.

btw, i've switched completely to the nursing pads and so long as i wipe the 'snot' off of it throughout the day i'm fine.  doesn't appear to smell.  put a folded up paper towel over the stoma at night so the pad is more fresh for the day.  still have way too much mucus, but that's minor compared to what you are going through .  janet

One thing my doc told me was that paralytic ileus has a high occurrence rate when it comes to KP surgery. I myself ended up with a nasogastric tube for a couple days due to non-stop nausea/vomiting (yuck!). One of the NPs involved with my care mentioned that chewing gum may prevent or shorten the duration of this. And a quick google will show there is medical literature that support this. I just wish I had known this early in the game. On that day I chewed gum, would't you believe it...my motility returned and my KP output started flowing like a waterfall. Just keep this in mind and good luck with your surgery!

So sorry Kara,

I know how hard on the morale redo's are but like you said, you are better prepared for this one.  

I was also shocked by the 33% statistic...I had never heard of that...is that his statistic or in general? I had enough redo's to impact that curve negatively...but I didn't know that so many other people had to suffer through them!

I just hope that this time things go well, you heal up quickly and that you are bouncing around with a happy and healthy pouch real soon.

As for the ileus, I am not sure that it is an obligatory thing...I had all of my k pouch surgeries done from 1979-2008 (8 or 9...I've lost count) and I never suffered from an ileus until 2009...my 10th k pouch surgery/revision...now I get them all the time along with blocages but they are not a given.

Evox, interesting about the gum chewing...I like the idea but my surgeon refused everything by mouth outside of ice chips...no chewing post k pouch surgery because he didn't want to stimulate the digestive system...Different doctors, different methods I guess...(I like yours better!)

By the way, in my 1st k pouch procedure they put a gastric tube directly into my stomach and not down my nose...it stayed in place for the full 30 days. (I have a cute little star shaped scar where they sutured it down)...they never used a NG tube in me or other patients...always directly placed it...not sure why.

Kara – – greetings from the silk Road in China. i'm sending you good thoughts and much healing white light as you head into surgery. You are in good care there a Cleveland clinic, how will I know. I don't like to wish good luck, to either you or Dr. Dietz, as it's skill not luck that's going to correct the valve – – yes?  I'm sure you will follow doctors and nurses orders and I do wish you the speediest of recoveries.

Hi Janet- at home now and very familiar with what I am feeling.  Pain is same and I have zero appetite.   I am not as nervous with the waters catheter so that helps.  Now 3 weeks of recovery and then back to work.  Thanks so much for checking in....

As a side note Dr. Dietz is leaving the Clinic as of August 5th- he is going to UH.  I am so disappointed.  It was mentioned on the forum previous and Dr. Ashburn will be taking all of his K-Pouch clients.  

Kara--my empathy that  you have to go through this in the heat of summer, besides twice.   Can understand why you don't have any appetite. I lost at least 15 pounds from my surgery and never gained it all back. Because I have to be so picky about  my food here I'm not eating enough.  I'll get back to my chocolate habit soon enough!

Have you considered switching to UH?   I would think Dr. Dietz would be pleased to have you continue to be his patient. It was really easy to get my records from Cleveland and then  send them to Dr. Kiran. I went to the extra effort of getting my records because I want to have my G.I. doc Boston also have a copy. CC would probably send them directly to UH and local doc.   I did write Dr. Dietz and the nurses notes that I was leaving and thanking  them for all their  help

Hi Jen- your so sweet to check up. All ok, everything same as before; just know (hope) my valve will be better and my Kpouch will function as it should. More mucous drainage than last time, and my staple incisions are a bit more tender, but I am moving along. How are you doing? Did you get all your to-go bags eventually sorted so you can go out with the myriad of supplies? �� Bummed Dr. Dietz will be gone and unsure if I will follow him to UH. He will do my follow up though at CC so glad about that! Please let me know how you are doing and thank you again for your kindness! Kara
