Hi- I hope someone has some ideas. On account of a bladder stone removal, and pieces they couldn't get out yet, I've got a catheter in there, (I'm male), which my bladder rejects and spasms badly over. My Dr gave me meds to relax the bladder muscles so they won't care so much about the catheter and it helps some, but when I try to empty j-pouch, the spasms come on full volume and it is very difficult to relieve the j-pouch because i have to put gentle pushing pressure which seems to work the bladder muscles as well and just gets them going. I've tried every position i can think of, and every mental trick to try getting my brain to give instructions only to the j-pouch, but nothing seems to help. I've avoided eating too much, but i know that if i could, there would be more to come out. I've also had less of a need to go and wondering if the alpha blocker relaxing the bladder is also relaxing the pouch. Is there anything i should be doing to avoid problems with the pouch over these next two days until they take this foley thing out? Thanks so much, rml

Well, for the record, and for "posterity"- that is - searches on this site for painful Foley Catheter and J-pouch, this is what I found. At least in me, activity in the pouch seems to trigger a bladder spasm which is very, very unpleasant. If the catheter is short term, i'd just drink to avoid the pain. Otherwise, eat, and when the time comes, (likely shortly after eating), ride out that pain seated, and as the pain subsides, engage muscles to relieve the pouch. Once the pouch is emptying, I found you can stay in, sort-of,  "pouch-muscle-mode" while the bladder waits for the pouch to do its thing. I get another bladder spasm when i get up, but at least the pouch is empty. Oh, there's more. Dr prescribed Tamsulosin, an alpha-blocker to help relax the bladder muscles. If you need, ask dr for twice per day as necessary. Note that Norco doesn't help a lot with that bladder pain but it does tend to constipate the pouch which in me made more pouch activity and hence, more bladder spasms. This week, I kept to foods that empty quickly for a change! Foley comes out tomorrow and stays out i HOPE! 

 I'm female and I cannot control things so that I either urinate or defecate.  I end up performing both functions at the same time......no matter what! After all of these years.....since 2002.......I've given up!  Easy for me as I'm seated on the toilet for both functions.  I don't think I would ever be able to urinate standing up with my J-pouch.  Only time I have a problem is when I have to given a urine sample.  I cannot give a "clean catch" unless I catheterize and I've forgotten how to do so!  Sometimes I just go to a urologist to have a nurse catheterize me.  Other times, I "stuff" my anus with cotton balls!  Sometimes it works and sometimes it doesn't.  Good luck!
